It is primarily renowned as the site of veneration for San Pio da Pietrelcina, the Capuchin friar who resided and passed away there in 1968. The sanctuary comprises several religious structures, including the lower church, which houses the tomb of St. Pius, and the upper church, a contemporary design by architect Renzo Piano, inaugurated in 2004.

Recognized as a UNESCO World Heritage Site, the cave is associated with a series of Archangel Michael apparitions dating back to the fifth century, establishing it as a major pilgrimage destination in Europe, particularly during the Middle Ages.
  Inside, the cave maintains a solemn and spiritual ambiance, enhanced by architectural elements accumulated over centuries: altars, statues, inscriptions, and profound faith testimonies. The shrine has been visited by popes, saints, and millions of faithful over the centuries, remaining a symbol of devotion and mysticism today.
